FHSAA Forms

Physicals cost a flat $25 per student; insurance is not required or billed. Cash and electronic payments are accepted on event day.

Event minimums target 20+ participating athletes within 15 miles of Orlando, with custom flexibility for your program.

Parents must pre-sign all required FHSAA forms; students bring them to the event for provider completion.
